Care Management

Nursing care management is a service provided by a Registered Nurse as they assist a client in navigating a particular health diagnosis. Care management services may include assessment of your current health and current lifestyle,  implementation of lifestyle supports, collaboration with multiple care providers, support with management of insurance challenges, planning for continuation of care, planning and preparing your environment for safe care, advocating for your health needs with your care providers, and more. 

Nurse care managers also act as Nurse Navigators when a client is presented with a new diagnosis. Perhaps you are not sure about which specialists you should see, or are interested in utilizing community resources to support your health and wellness. Nurse care managers support your health and wellness by connecting you with community resources and assisting clients in implementing lifestyles changes.

Our admission process begins with our signature NEST assessment. This includes assessment of your Nutrition, Environment, Supports, and Tasks. Our nurses assess your health, wellness, and current lifestyle to make certain we help you reach your health goals. Nestcare nurses make certain to support you living well at home. This includes crafting a personalized care plan to support the health of your brain and body. Our care plans are stored in your electronic record and are available for your healthcare provider as well. We communicate with any providers at your request to build the most comprehensive care plan possible.
